Disability Insurance

Apex offers you two disability plans that work together to keep part of your paycheck coming if you cannot work because of illness, injury or pregnancy. Disability benefits are administered through Unum.

Apex provides you with STD and LTD benefits at no cost to you.

Short-Term Disability

Short-Term Disability Benefits
Weekly Benefit	60% of weekly earnings
Maximum Weekly Benefit	$1,000
Benefits Begin On	15th day (accident) 15th day (illness)
Benefit Duration	11 weeks until LTD begins
Pre-existing Condition Limitation	None

Long-Term Disability

Long-Term Disability Benefits
Benefit Increments	60% of monthly earnings
Maximum Monthly Benefit	$5,500
Benefits Begin	91st day
Actively at Work	You must be actively at work on the effective date of coverage
Pre-existing Condition Limitation	If treatment is received 3 months prior to your effective date, then the condition will not be covered until you have been covered for 12 months
Mental/Nervous Illness	24 months
Substance Abuse	24 months
